Dr. Cooke has no contract with any insurance company. She is a founding member of the Independent Doctors of New York (www.idny.org), a group of board-certified physicians completely independent of managed care contracts.

For Internal Medicine services, the cost of consultations is variable as per Health Care Finance Administration (HCFA) guidelines, depending upon the complexity of clinical decision making, the time spent, and the nature of the service rendered (clinical consult, hypnosis, preventive counseling, EKG, etc.). Typically, the initial medical consult, lasting 75 minutes, is billed for $650. An initial consult lasting as long as two hours will be billed for $700. Please inform the office if you anticipate or desire extended consultation. A detailed follow-up consultation of 30 minutes is billed for $200. A schedule of fees is available upon request.

Regarding acupuncture charges, the initial consultation will be billed for $400 for a one hour visit which includes an expanded problem-focused history and physical examination and initial acupuncture treatment. Follow-up acupuncture treatments range between $150 and $200 and may include adjunctive therapies such as moxabustion, infrared or electrical stimulation. If additional consultation time is taken for other clinical review, the consultation charge will be added to the acupuncture fee.
Payment is requested at the time of each visit.

Additionally, long distance telephone consultation is available for non-Medicare patients. However, these fees are not billable to insurance. Please contact the office by email or telephone for terms of this type of consultation.
