Once you have made an apppointment with the doctor, you will be given a special password for download of the forms attached to this page. Please fill them out prior to the time of your appointment. Bring them with you to the first appointment. This is sure to save time. If you choose to send the completed forms by email, only be aware that Dr Cooke cannot guarantee inviolability of the forms to internet hackers.
